Having been appointed to work as the program coordinator in Health for All Agency, my
task is to initiate programs that drive the agency’s mission of improving health care. The
program on Home Health Care and Nursing seeks to decrease medical costs that arise from
prolonged medical stays in hospital by providing health care at home, thus reducing the duration
of health facility stays. The effect is to promote of practices that will result in discharge of
numerous patients and linking them to home health care for continued monitoring of their
conditions. Currently, home health care is a globally acknowledged approach for attaining a costeffective system that promotes patient fulfilment, a reduced burden on the occupancy rates in
hospital beds as well as reducing the rates of hospital infections (Institute of Medicine, 2015). In
the expansion of the project, $10,000 funding is requested to meet the needs of staff training and
resource materials that are required for this program.
Statement of need
The Health for All Agency is seeking a grant to develop the Home Health Care and
Nursing program covering the Westminster hospital. This particular hospital has a bed capacity
of 1500, with over 700 expected to spend prolonged durations in the hospital. This puts them at
risk of incurring hefty medical costs that may put them in financial crisis. Among these patients
are the elderly, who suffer from a myriad of chronic diseases. The various conditions to be
covered for home health care include cancer, diabetes, hypertension, cardiovascular diseases and
other chronic infections. If such patients are not offered the alternative of receiving health care at
home at reduced costs, they are likely to suffer massive financial implications.
Program description
The Home Health Care and Nursing program will enable patients suffering from chronic
illnesses to acquire the same quality of health care at home. Through the use of home care nurse
practitioners and physicians, the program will deliver healthcare services at the home level. For
most of the time, this mission will be accomplished with joint efforts from home health
professionals to form an all-encompassing interdisciplinary team. The major aim of home health
care is to supersede hospital admittance for the aforementioned category of patients through the
provision of intensive care at home in a similar manner to hospitals. Individuals in receipt of the
home health care at the utmost acuity level will incur lower health care expenditures, with
greater satisfaction, and equal or better outcomes when paralleled with alike inpatients.
The needed task force
Clinical teams comprising of healthcare professionals across various disciplines such as
medical occupational therapists, social workers, home health aides, nurses, and physical
therapists among others will be incorporated. As the life expectancy increases, the elderly
people show disease susceptibility from the effects of a deteriorating immune system. In fact,
Landers et al., 2014 states that growing numbers of elderly people “will live with multiple
chronic conditions, such as diabetes or dementia, and functional impairments, such as difficulty
with the basics of life like mobility and managing one’s household”. As such, the major
challenge for the health care industry is in making certain that elderly people with severe chronic
ailments and other disorders that come with aging remain independent. At this point is where the
home health care and nursing comes in to fill the gap.
Notably, home health care and nursing provides a variety of medical services in the same
capacity as in hospital settings. Other than these wide-ranging health services along home health
care, it is as well essential to make mention of the massive role played in offering palliative care
for the elderly. For individuals who have been diagnosed with severe and life threatening
illnesses such as cancer, palliative care is constantly a core component of patient treatment in an
all-inclusive approach that puts emphasis on bodily functions. Currently, Medicare allows
palliative care to be delivered outside of the hospital settings enabling the patients to access
home health care and nursing.
It is evident that patients have a preference towards obtaining health care while at home.
The Alliance for Home Health Quality and Innovation, 2014 states that “surveys of older
Americans have found that most prefer to stay in their own homes”. Furthermore, the economic
trend is largely towards on-demand, personalized, and direct-to-consumer services. As such, this
program through the agency will specialize in the provision of skilled, home based nursing and
therapy services to patients who are homebound. Some of the services will target patients who
have gone through hospitalization and are recuperating from severe infections or conditions. The
program will also target patients who require community-based care to manage their prolonged
health conditions.
Goals & Objectives
The program on Home Health Care and Nursing is aimed at decreasing medical costs that
arise from prolonged medical stays in hospital by providing health care at home.
The objectives include:
1. Providing a quantifiable decrease in hospital occupancy in the Westminster facility. The
objective is that the patients with chronic illnesses will be discharged through our program.
2. Enabling the elderly to acquire medical services at home through the use of a team of clinical
experts such as nurses, physicians, and others.
3. Providing the terminally ill patients with an alternative that will help them receive health care
while surrounded by their family members.
