Hourly Rounding Picot
The PICOT format is a model used to frame clinical questions and assessing them more
quickly and efficiently. PICOT is an acronym to represent; P- problem, patient or population, IIntervention, C- comparison, O- outcome and T- timeframe. Using these elements, we can be able
to deduce a clinical question to get the results needed. Our topic is; hourly rounding as a quality
improvement and safety suggestion. Using PICOT format, we can phrase it as; in patients admitted
in hospitals, does making hourly rounds as compared to not making these rounds, improve the
quality of services rendered and safety over the period admitted in the hospital? It is a clinical
problem that we have to solve.
Hourly rounding can be explained as the systematic nursing intervention which is designed
to address the needs of admitted patients. Trials conducted by the Asian Nursing Research (2014)
whereby they split patients in surgical ward into groups and then performed routine checkups on
one team than the other. Their findings were that the association with regular checkups had
significant patient satisfaction as compared to the other group and patient satisfaction has become
an outcome indicator in determining the quality and efficiency of healthcare units. (Merkouris
et al., 2013).
A nurses role is crucial in a healthcare facility, in fact, Han, Connolly, & Canham ( 2003)
state that satisfaction with nursing care is the most critical index in determining patient satisfaction
and the overall care provided. A nurse intervention is defined as any treatment that is based on
clinical knowledge and judgment performed by a nurse and should enhance patient outcomes.
Studies have shown that patients with the uncertainty of when nurses are available or when they
can be able to tend to them, they worry and could cause the rise in anxiety levels and could start
showing inappropriate coping mechanisms (Anon 2017). Hence a structured format with regular
checks would serve a significant number of patients and go a long way in patient satisfaction.
Patient satisfaction is crucial in hospitals to receive reimbursements and good ratings. In
order to advance patient happiness, inventiveness such as preparation courses and training plans
have been introduced to show the nurses about the importance of connecting with patients (Toma,
Triner, & McNutt, 2009). For efficient patient care, one has to have clinical knowledge and
understanding of issues and affect the delivery of care. Nurses are best suited for this since they
spend the most time with patients. Improving health care is a priority and hence could be argued
that increasing rounds in hospitals are vital in making this a reality. It will guarantee better patient
care.
Health care agencies oversee a wide range of department divisions in hospitals such as per
diem healthcare staffing services or direct placement. Hundreds of nurses are placed in hospitals
to provide adequate care to patients. Nursing practice is a profession in the healthcare industry that
is tasked with taking care and aiding of patients and the community at large. Nurses play critical
roles as stated above and their actions make up the nursing practice. Regular or hourly nursing
rounds should also be made part of this to improve universal health care.
In conclusion, in healthcare centers where regular rounds are not done, patient satisfaction
is quite low, and implementation of hourly nursing rounds will have positive effects on patient
satisfaction of as compared to early irregular rounds. This method is here to improve patient-nurse
interactions and by doing so upgrade the quality of nursing care as well as patient satisfaction in
the duration that the patient is in the healthcare facility. We have utilized the use of PICOT in the
above paper and implemented as it should.
